The following forms have been prepared by the Manitoba Labour Board to assist parties in the filing of various applications. Printed forms are also available from the Board's office.

Applications must be filed in accordance with the Manitoba Labour Board Rules of Procedure. If you have questions regarding the Rules or the filing of your application, you may contact a Board Officer at 204-945-3783.

The Manitoba Labour Board accepts applications / complaints through e-mail, to MLBRegistrar@gov.mb.ca, or in person. The hours for filing forms or applications with the Board are from 8:30 a.m. to 4:00 p.m. Monday to Friday, excluding statutory holidays. Please be advised that any correspondence received outside of these hours shall be deemed to be received on the next business day.
